To designate both Social Security and Medicare withholding:

Select Medicare Deduction.
Select Social Security Deduction.

To designate Medicare only withholding:

Select Medicare Deduction.

To designate the Social Security only withholding option:

Select Social Security Deduction.

To not withhold FICA:

Click on box next to either deduction in order to remove the X from the box.
